On or about September 15, 2015 located at the 3461 Windisch Ave. job site a crane operator performing hoist tensioning and winching of a sling rigged downed tree section with a Manitowoc Manitex M2892S truck mounted crane, was exposed to falls from the crane and struck-by hazards from structurally damaged falling residential framing components due to crane failure during load contact tensioning against obstructions in the load travel path.
