On or about September 15, 2015 located at the 3461 Windisch Ave. job site a crane operator performing hoist tensioning and winching of a sling rigged downed tree section with a Manitowoc Manitex M2892S truck mounted crane, was exposed to falls from the crane and struck-by hazards from structurally damaged falling residential framing components due to crane failure during load contact tensioning against obstructions in the load travel path.

29 CFR 1910.184(i)(9)(iii):  Synthetic web slings with snags, punctures, tears or cuts shall be immediately removed from service:    On or about September 15, 2015 located at the 3461 Windisch Ave. Cincinnati, OH job site a mobile commercial truck mounted boom Manitowoc Manitex M2892S crane, was performing hoist tensioning and winching of a downed tree section rigged approximate six foot endless type v synthetic web sling with snags, punctures, tears or cuts which was not immediately removed from service.
