29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: a) The employer did no ensure that an adequate hazard assessment was conducted for employees performing tree trimming activities in that on or about September 16, 2015, an employee was cut while utilizing a chain saw and was not wearing personal protective equipment to protect the arms.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: a) Located on the front exterior side of the commercial building workers were engaged in tree trimming activities. Employees were exposed to potential amputation and struck-by hazards in that the workers were not properly trained in the proper use of personal protective equipment such as arm, eye, and head protection and how to utilize the proper PPE that will protect the affected employee(s) from the hazards identified in the hazard assessment.
