29 CFR 1926.152(a)(1): Containers other than approved metal safety cans or DOT approved containers were used for the handling and use of flammable liquids in quantities of 5 gallons or less:  On or about 9/18/2015 at 5455 Enfield Way, Suwanee, GA: employees performing refueling of the Jenny Air compressor were exposed to thermal burns where the safety can cap was missing and a makeshift cover was made from foam building materials.  29 CFR 1926.155(l): "Safety can" means an approved closed container, of not more than 5 gallons capacity, having a flash-arresting screen, spring-closing lid and spout cover and so designed that it will safely relieve internal pressure when subjected to fire exposure.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about 9/18/2015 at 5455 Enfield Way, Suwanee, GA: employees performing paper and shingle installation were observed:  a. descending from the roof in order to get a square of shingles the worker was wearing rock-climbing harnesses and a full body harness and failed to connect on a 10/12 pitched roof, upon ascending back onto the roof after picking up the square of shingles exposing him to falls of up to 34' to the lower level from the peak and 22' from the eves.  b. sitting on the ridge not wearing any fall protection at all on a 10/12 pitched roof exposing him to falls of up to 34' to the lower level from the peak.  Ignacio Carrera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 960881, citation number 1, item number 1 and was affirmed as a final order on 4/15/2014, with respect to a workplace located at 14339 Highway 92, Heron Pond Lot #50, Woodstock, GA.
