29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. On or about September 16, 2015, the employer failed to protect employees from a tipping hazard when the employees were required to drive powered industrial vehicles through the east entrance bay. The concrete at the immediate entrance of the bay was uneven with large cracks and pot holes that were approximately 4 inches deep that could cause the operator to tip or drop the load.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. On or about September 16, 2015, the employer failed to protect employees from a struck by hazard when it used the top of the supervisors office on the west side of the plant for storage of building materials without load rating the structure and posting the maximum approved load.
