29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards identified in the hazard assessment: (a) Thermite Area - The employer did not provide adequate protective equipment for an employee who received 3rd degree burns on the back side of his legs when the thermite reaction exploded during the ignition process. The employee wore chaps that covered the front part of his legs and not the entire leg portion, on or about September 18, 2015. No Abatement Certification or Documentation Required.
