29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation: North Franklin, Connecticut: The OSHA 300 Log for the year 2015, was not completed as detailed per the regulation. The following deficiencies were found: A. Case Number 1, which occurred on August 20, 2015, concerning an employee who sustained a leg injury, did not have the location properly identified. B. Case Number 2, which occurred on August 5, 2015, concerning an employee who sustained an eye injury did not have the location properly identified.
