29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a)  Roof, 100 Pearson Miller Drive, Pompton Lakes, NJ: Employee was not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate the hazard. On 9/18/2015 an incident occurred where an employee fell approximately 16 feet resulting in serious injuries.
