(a) On or about 09/21/2015 - at 2765 Brown Street, areas, such as but not limited to, the west wall at the Ruvo door machine and casing assembly area, the door light machine and saw area, and the lower building at the glass storage area and air compressor room were not kept clean and orderly due to millwork stacked on the floor and work tables and leaning against the walls and extensions cords, loose floor tile, packaging materials, and trash on the floors creating tripping, slipping, and struck-by hazards.    (b) On or about 09/21/2015 - at 2765 Brown Street, along the outside of the east wall obsolete materials and packaging materials were haphazardly stored and there was no vegetation control creating tripping hazards.    (c) On or about 09/21/2015 - at the Lower Building, in the Glass Storage area the south and east walls were not kept in a sanitary condition in that mildew was observed in large areas in multiple sections on the walls creating respiratory hazards.
