29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Jobsite - On or about September 16, 2015, an employee not wearing a hardhat was picking up discarded shingles in an area where they were being thrown off the roof and was exposed to the hazard of a head injury from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  Jobsite - On or about September 15, 2015, an employee was exposed to a fall hazard of approximately 11 feet on a steep residential roof without fall protection.  The employee had removed shingles on the roof of a dormer area of the roof that could not have been removed without being exposed to the fall hazard.
