29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On September 9, 2015, the Bread Packaging Machine air pressure valve was being adjusted by reaching underneath machine where in running chains and sprockets were located, exposing employees to an amputation and laceration hazard due to not affixing a lock or tag to each energy isolating device. No documentation and certification required for this item.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: On September 9, 2015, an employee performing servicing and maintenance activities such as, adjusting an air pressure valve underneath the Bread Packaging Machine, was exposed to an amputation and laceration hazard to hands and fingers. The employer did not ensure that lockout/tagout training was provided to all authorized and affected employees to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
