The process did not include such items as, but not limited to, high and low temperatures of pumps, on or about September 22, 2015. (b) 817 Spangler Road, Camp Hill Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process did not include such items as, but not limited to, failure of the nitrogen system, on or about September 22, 2015. (c) 817 Spangler Road, Camp Hill Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process did not include such items as, but not limited to, the failure or improper positioning of the hand valves, control valves, and relief valves, on or about September 22, 2015. (d) 817 Spangler Road, Camp Hill Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process did not include such items as, but not limited to, failure of the Scully interlock system, on or about September 22, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
