On 9/18/15, at the Dacula facility, the employer did not ensure the supplemental form used for injuries and illnesses on the 300 log contained equivalent information as what is contained on the OSHA 301 form. The following information was not included on the form the employer used: Date of hire, physician name, name of treatment facility, if treated in an emergency room, if hospitalized, the case number on the 300 log, time employee began work, what the employee was doing just before the incident occurred and who completed the form.
