29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  On 9/18/15, at the Dacula facility, employees using corrosive chemicals (F 362 No Rinse Sanitizer and Ecolution Pro Degreaser Cleaner) were not provided an eye wash in the kitchen area where they are used.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.  On 9/18/15, at the Dacula facility, the employer did not provide a training program to employees in the use of fire extinguishers when employees were expected to use extinguishers on incipient level fires.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information.  On 9/18/15, at the Dacula facility, the employer did not ensure employees were trained in a Hazard Communication program that included the Globally Harmonized System information as well as updated safety data sheets that employees were expected to use, including Ecolotion Degreaser and Avance Sani Quat).

On 9/18/15,  at the Dacula facility, the employer did not ensure column F on the OSHA 300 log was filled out in the detail as required by the regulation for description of the injury. Instead, for injuries occurring in the last 6 months, descriptions listed the following: back and hip strain, hip and ankle bruise, singed eye lashes and hair and right forearm strain.

29 CFR 1904.29(b)(4): A form with equivalent information to OSHAs 301, Injury and Illness Incident Report, was not used by the employer to document each recordable injury and illness entered on the OSHA 300 Log.  On 9/18/15, at the Dacula facility, the employer did not ensure the supplemental form used for injuries and illnesses on the 300 log contained equivalent information as what is contained on the OSHA 301 form. The following information was not included on the form the employer used: Date of hire, physician name, name of treatment facility, if treated in an emergency room, if hospitalized, the case number on the 300 log, time employee began work, what the employee was doing just before the incident occurred and who completed the form.
