29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized:    a) Construction site, 2nd floor kitchen - On or about August 11, 2015, while completing wiring of the power supply circuit for the soup wells, an employee was stripping insulation from one leg of the 208V circuit which had not been locked/tagged out, as the employer did not effectively enforce the locking and tagging of circuits resulting in the conductor being inadvertently energized, resulting in the employee receiving a severe electric shock.
