OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:, in that employees were exposed to crushing hazards:      (a) Dauphin County Prison, 501 Mall Road, Harrisburg, Pa 17111- Mustang 1155 telehandler was not equipped with a seat belt restraint system.  Employees operated the telehandler without wearing a seatbelt on or about September 22, 2015.       Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines:    a) Dauphin County Prison, 501 Mall Road, Harrisburg, Pa 17111 - The employer did not ensure that a path formed by two warning lines was established between the access ladder and the work area on or about September 22, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    (a) Dauphin County Prison, 501 Mall Road, Harrisburg, Pa 17111 - Employees were observed using a fiberglass ladder that did not extend at least three feet above the landing surface to the building, thereby exposing employees to a fall hazard of approximately 19 feet to the ground below, on or about September 22, 2015.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    (a) Dauphin County Prison, 501 Mall Road, Harrisburg, Pa 17111 - An employee worked near the edge of the roof and was exposed to a 15 foot fall hazard. Employee was beyond the warning line system without fall protection or a safety monitor on or about September 22, 2015.     T.G.W Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 411342, citation number 1 item number 1 and was affirmed as a final order on June 28, 2012 with respect to a workplace located at 1000 S 8th Street, Lebanon, Pa, Lebanon High School, Library Wing, North Side.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
