On or about 08/27/2015, inside the Fabrications area, employee operating a Cincinnati Hydraulic Press Brake, m/n 90-350 CB II, s/n 51863, engaged in over break operations with his left hand in the point-of-operation, resulting in the amputation of his ring finger from the tip to the first joint.
