29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Work floor - On or about September 17, 2015, employees were wearing a body belt while operating a high lift powered industrial truck from its elevating control station, exposing the operators to a fall hazard routinely exceeding four feet above the lower level. b) Work floor - On or about September 17, 2015, employees were using a retractable lanyard, while operating a high lift powered industrial truck from its elevating control station at heights greater than four feet above a lower level, which had a knot tied in the lanyard's line, which may reduce the effectiveness of the fall arrest system in the event of an activation.
