On or about 9/22/15, on site, for employees manufacturing duct work, an employee was injured when a flying particle got into his eye. The employee operates the plasma cutter. The employer did not assess the workplace to determine if hazards were present to require the use of safety glasses. Safety glasses were provided for voluntary usage.

The employer did not establish and implement elements of a written respirator program that relate to the medical evaluation of respirator users, the cleaning, the storage, and the maintenance of respirators.
