29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions. On or about September 17, 2015 and at times prior thereto, employees were not wearing appropriate personal protective equipment that would protect them from chemicals such as but not limited to ethanol, quartz, and carbon black. This condition exposed employees to health hazards.
