29 CFR 1926.451(f)(6): The clearance between scaffolds and high-volt uninsulated power lines was not a minimum of 10 feet when scaffolds were erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer to exposed and energized power lines:   a)  79 N Beverwyck Rd., Parsippany, NJ   An employee erected a scaffold within approximately 2 feet 1 inches of energized high voltage power lines.      Violation occurred on or about 08/14/2015.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a)  79 N Beverwyck Rd., Parsippany, NJ   Employees were exposed to an approximately 12-foot fall hazard to the ground as the worker access the roof to erect the scaffold.    Violation occurred on or about 08/14/2015.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a)  79 N Beverwyck Rd., Parsippany, NJ   Employees were exposed to an approximately 10-foot fall hazard to the ground as the ladder side rails did not extended at least 3 feet above landing surface.   Violation occurred on or about 08/14/2015.
