Miller Mill Co., Inc. :  On or about September 17th, 2015, and times prior to, the employer exposed  employees to blood borne pathogen hazards by failing to conduct annual training for employees on a written Blood Borne Pathogen Plan to include an Exposure Control Plan was implemented to cover all employees with exposure to blood or other potentially infectious materials to include employees that cleaned up blood from an forklift accident that resulted in laceration to top of left hand, broken left hand ring finger and crushed left hand pinky finger.  No training record for 2015 could be produced for Employee #1.   b)      T. R.
