29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: A) Con-way Freight, Inc., located at 1850 East Wyandot Avenue, Upper Sandusky, OH 43351: On or about June 24, 2015, the employer did not notify the Occupational Safety & Health Administration within twenty-four hours after an in-patient hospitalization and amputation injury. The delay in reporting of the in-patient hospitalization, amputation, or loss of an eye to OSHA can result in the delay of the incident investigation and identification of hazards.
