29 CFR 1910.23(a)(3)(i): 29 CFR 1910.23(a)(3)(i):  When a floor opening was not in use, the hinged floor opening cover was not closed or the exposed side was not guarded at both top and intermediate position by removable standard railings:    On or about June 19, 2015, the employer failed to ensure the hinged floor opening cover on the bridge of the Gantry Crane #5 was closed at the bottom of the ladder (first ladder access to the bridge entering from the roof, traversing downward to the bridge crane).  Each additional ladder descending downward towards the bridge also had their hatch in the open position.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide:    On or about June 19, 2015, the employer failed to ensure that a hole which was located at the base of the top ladder of Gantry Crane #5 from the roof access was covered or repaired.  The hole measured 4 x 11 inches and was located just below the last rung of the access ladder and right at the edge of the hatch opening.
