29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) 311 Broad St, Chattanooga - On or about 9/21/15 employees were exposed to fall hazards when the employer did not instruct employees in the recognition and avoidance of hazards associated with moving and removing sidewalk grating. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) 311 Broad Street, Chattanooga - On or about 9/21/15, employees were exposed to fall hazards of approximately 18 feet through sidewalk grates when they were not protected from falling through. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
