29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place: a) Cooper University Hospital One Cooper Plaza, Camden, NJ 08103: The employer did not ensure that employees with ongoing contact with patients or blood and were at risk for percutaneous injury obtained a hepatitis B titer to assure immunity as recommended by the U.S. Public Health Service, on or about 1/8/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
