29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 08/31/2015, at the jobsite at 1722 Schnarre Road, Foristell, MO, employees installing and standing on trusses were not provided with conventional fall protection. Two of the five employees were standing on the top of the truss which was 12' high. The total fall distance from the top of the truss to the ground was approximately 25 feet. The other three employees, who were standing on the top of the wall, were exposed to a 13' fall to the ground.

29 CFR 1926.503(a)(1): The employer does not ensure that a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about 08/31/2015, at the jobsite at 1722 Schnarre Road, Foristell, MO, two employees engaged in truss installation operations were exposed to an approximately 25 feet fall. Three other employees who were also engaged in truss installation operations were exposed to a 13 feet fall hazard. Fall protection was not utilized and the employer failed to provide the employees with fall protection training.
