29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:    a)  Stairway to the Second Floor Agitator Room - On or about September 23, 2015, employees, using the stairway to access the agitator room, were exposed to a fall hazard of 8 feet because the wall side of the stairway was not equipped with a standard railing as defined in 29 CFR 1910.23(e)(1).  There was no mid-rail on the wall side of the stairway where the wall panel had been removed, exposing employees to a fall hazard of 8 feet into a flour hopper.     No abatement certification or documentation required.

29 CFR 1910.119(d)(3)(i): Process safety information pertaining to the equipment in the process did not include the elements specified in 29 CFR 1910.119(d)(3)(i)(A) through (H):    a)  For the Flour Bleaching Process - On or about September 30, 2015, the employer did not compile complete and detailed process safety information (PSI) to meet the requirements of 29 CFR Part 1910.119(d)(3)(i)(A) through (H) before conducting the process hazard analysis in that the piping and instrument diagram did not  have a legend to define the symbols nor was it labeled with the specific identity of the pressure relief valve in use; the pressure relief valve system design, design basis, and PSI were not readily available; and the material and energy balances for the process had not been determined.      Abatement certification required within 10 days after abatement date. 29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph:    a)  For the Flour Bleaching Process - On or about September 30, 2015, the employer's process hazard analysis [PHA] did not to meet the requirements of 29 CFR Part 1910.119(e)(3)(i) through (vii) in that it failed to identify, evaluate, and control the hazards of the process (nitrogen trichloride potential), the degree or range of the safety and health effects of chlorine on employees or the number of employees upon which the failure of controls or the consequences of a deviation might have an impact, or the hazards related to facility siting in that the growing plant had encompassed the existing Flour Bleaching Process, which exposed more employees to small-scale releases and minor equipment failures.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1910.134(h)(1)(ii): Respirators issued to more than one employee were not cleaned and disinfected before being worn by different individuals:  a)  For the Flour Bleaching Process - On or about September 30, 2015, the employer had not implemented respirator work rules which met the requirements of 29 CFR 1910.134 Mandatory Appendix B-2 or were at least as protective to ensure the respirators were properly cleaned and disinfected in a manner that prevented respirator damage and user harm in between different respirator users for employees being exposed to chlorine respiratory hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
