29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section: On or about 09/24/2015, PIT's (#s: 48, 49, 58) have been altered without manufacture's permission with regard to moving the horn button from the steering wheel to the side of the steering columns or behind the driver's seat making it difficult for the operator(s) to safely operate these fork trucks and alert employees at the corners, blind spots and intersections. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheets are enclosed with the citation(s).

29 CFR 1910.335(b)(1): Safety signs and tags. Safety signs, safety symbols, or accident prevention tags shall be used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145: The electrical equipment throughout VMF and P & DC are not properly labeled with OSHA EWP Labels. There were two different labels being utilized at the time of the inspection. 1910.145 states that there shall be no variations in the type of design of signs posted to warn of specific hazards. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheets are enclosed with the citation(s).
