29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a) On the elevated platform above the Salfex mixers at 730 Worcester Street Springfield, MA: On or about September 21, 2015, two floor holes, measuring approximately twenty-six inches long by ten and one-half inches wide and twenty-five and one-half inches long by sixteen inches wide, were not guarded with a standard guardrail and toeboard. b) On the elevated platform above the gradient mixer and bagger at 730 Worcester Street Springfield, MA: On or about October 15, 2015, floor holes around the gradient mixing and bagging machine, measuring approximately twenty-seven inches long by nine inches wide and forty-two inches long by six inches wide, were not guarded by a standard guardrail and toeboard.
