29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  a) On the elevated platform above the Salfex mixers at 730 Worcester Street Springfield, MA: On or about September 21, 2015, two floor holes, measuring approximately twenty-six inches long by ten and one-half inches wide and twenty-five and one-half inches long by sixteen inches wide, were not guarded with a standard guardrail and toeboard.  b) On the elevated platform above the gradient mixer and bagger at 730 Worcester Street Springfield, MA: On or about October 15, 2015, floor holes around the gradient mixing and bagging machine, measuring approximately twenty-seven inches long by nine inches wide and forty-two inches long by six inches wide, were not guarded by a standard guardrail and toeboard.

29 CFR 1910.24(f): Rise height was not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:  On the stairs leading down to the grinders at 730 Worcester Street Springfield, MA: On or about September 21, 2015, the rise height of the top stair was not uniform throughout the flight in that the top step rise height was measured at approximately thirteen (13) inches with the remaining stair rise height measuring approximately seven (7) inches.

In the facility at 730 Worcester Street Springfield, MA: On or about October 8, 2015, the employer had not administered a hearing conservation program for five employees whose 8-hour, time-weighted average noise exposures were measured between 85.2 and 90.7 decibels which exceed 85 decibels measured on the A scale.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) In the loading dock area outside the office at 730 Worcester Street Springfield, MA: On or about September 21, 2015, a flexible extension cord was used as a substitute for fixed wiring to supply power to a Weigh-Tronix model WI-125 pallet scale.  b) Near the extruding end of the Saflex line at 730 Worcester Street Springfield, MA: On or about October 8, 2015, a flexible extension cord was used in lieu of permanent wiring to supply power to a Flotec model FPOS2400A-09 submersible pump used in the Sweco liquid and solid separator process.
