29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   (a) On or about August 13th, 2015, and at certain times before, employees in the Schlatter Department operating the Standard Resistance Welder (Serial #1931) , and similarly configured resistance welders, were exposed to possible hand and arm injuries in that the two-handed control devices were not designed and constructed so as to prevent the operator from having any part of their body in the danger zone during the operating cycle.  On August 13th, 2015, an employee was observed activating both hand control devices with one forearm while holding down the work piece with his other hand which was in the danger zone.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
