29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about September 28, 2015 at the jobsite located at or near 3203 NE 92nd St. in Kansas City, MO employees were exposed to an eye injury in that they were using tools without protective eye equipment.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 28, 2015 at the jobsite located at or near 3203 NE 92nd St. in Kansas City, MO  employees were exposed to a fall hazard in that they were working on a steep roof without an approved form of fall protection.
