29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a) On or about September 28, 2015, the employees were exposed to fall hazards greater than 6 feet due to a portable ladder that did not extend at least 3 feet above upper landing surface. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes:  (a) On or about September 28, 2015, the employer did not protect employees from caught-in hazards due to the air compressor not being guarded. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about September 28, 2015, the employees were exposed to injuries from falls approximately 15.0 feet while engaged in roofing activities.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  EP Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1050482, citation number 1, item number 1 and was affirmed as a final order on May 11, 2015, 2015, with respect to a workplace located at 3612 S. Jebel Cir., Aurora, Co 80016.   EP Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1021503, citation number 1, item number 1 and was affirmed as a final order on February 10, 2015, with respect to a workplace located at 10458 East Prentice Place, Englewood, Co 80111.
