(a) On or about 10/01/2015 - at Slitter #2, the energy control procedure LOTO-SL2-003 for Mill 5 Slitter did not clearly and specifically identify all of the hazardous energy sources to be locked out by employees to isolate and disable the electrical sources to the slitter line.
