At the jobsite, the portable extension ladder used to access the roof was not secured from displacement, exposing employees to a 19-20 foot fall hazard, on or about 9/30/2015.

On the 6/12 pitched roof, fall protection was not provided to employees installing shingles, exposing the employees to a 19 foot fall hazard, on or about 9/30/2015.    C. Sterling Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1008081, citation number 1, item number 1 and was affirmed as a final order on 12/15/2014, with respect to a workplace located at 11635 Brighton Knoll Loop, Riverview, FL 33579.    C. Sterling Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315481119, citation number 2, item number 1 and was affirmed as a final order on 12/21/2011, with respect to a workplace located at 6904 Roundleaf Dr, Jacksonville, FL 32258.    C.
