29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Jobsite:  Extension ladder at front of building      The extension ladder used by subcontractor employees to access the roof did not extend at least 3 feet above the eave and the ladder was not secured to provide its displacement.

29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces:  Jobsite:  Extension ladder at front of house.  The extension ladder's feet rested on shingle debris that could cause the ladder to slip.
