29 CFR 1910.303(b)(6): Conductors and equipment were located in damp or wet locations: a) Wall near abandoned restroom, 3rd floor of BOP shop, Braddock Avenue and 13th Street, Braddock, PA 15104: Observed on or about November 11, 2015, energized electrical panels and switches (480 volt, 3 phase) were located in a wet location. Water from leaking from pumps on a higher floor was accumulating on and near the electrical panels and switches. Employees working on the maintenance and winterization crews were exposed to the hazards of fire and electric shock.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and/or cables were attached to building surfaces: a) 3rd floor of BOP shop, near the "F" vessel hood, Braddock Avenue and 13th Street, Braddock, PA 15104: Observed on or about November 11, 2015, a flexible cord (110 volt power) was attached to building surfaces at various locations along its length. The cord was being used to supply power to electrical equipment on the third floor of the BOP shop. Employees working on the maintenance and operations crews were exposed to the hazards of fire and electric shock. b) 3rd floor of BOP shop, near the "R" vessel hood, Braddock Avenue and 13th Street, Braddock, PA 15104: Observed on or about November 11, 2015, a flexible cord (110 volt power) was attached to building surfaces at various locations along its length. The cord was being used to supply power to electrical equipment on the third floor of the BOP shop. Employees working on the maintenance and winterization crews were exposed to the hazard of fire and electric shock hazards. b) 3rd floor of BOP shop, near "F" vessel hood, Braddock Avenue and 13th Street, Braddock, PA 15104: Observed on or about November 11, 2015, the metal lamp shade on a portable hand lamp being used to keep a pilot control valve warm was not grounded. Employees working on the maintenance and winterization crews were exposed to the hazard of fire and electric shock hazards.
