29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:    a) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in LAN room area, three (3) relocatable power taps were not directly connected to a permanently installed branch circuit receptacle.   b) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in workstation #33 area, two (2) relocatable power taps were not directly connected to a permanently installed branch circuit receptacle.    c) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the Assistant District Manager's office, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle.  d) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the training room area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle.  e) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in workstation #12, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle.   f) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the Assistant District Manager's office area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle.  h) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in workstation #4, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle.   i) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in workstation #27, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle. 29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops shall have ground-fault circuit-interrupter protection for personnel:  a) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the female bathroom area, electrical receptacles were not equipped with ground-fault circuit-interrupter protection.  b) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the male bathroom area, electrical receptacles were not equipped with ground-fault circuit-interrupter protection.  c) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the female bathroom of the lobby area, electrical receptacles were not equipped with ground-fault circuit-interrupter protection.  d) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the male bathroom of the lobby area, electrical receptacles were not equipped with ground-fault circuit-interrupter protection. 29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:  a) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the training room area, an electrical outlet box was not covered or closed.  b) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the LAN room area, two (2) electrical outlet box were not covered or closed.  c) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, between workstation #27 and #28 area, an electrical outlet box was not covered or closed.  PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.305(g)(1)(iv)(C): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used where run through doorways, windows, or similar openings:   On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, between workstation #27 and #28 area, a flexible cord was run through wall opening.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
