In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear: a) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the training room area, an electrical outlet box was not covered or closed. b) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, in the LAN room area, two (2) electrical outlet box were not covered or closed. c) On or about 9/23/15, at 2190 Ponce-By-Pass, Ponce, PR, between workstation #27 and #28 area, an electrical outlet box was not covered or closed.
