29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On September 11, 2015 at the workplace located at 5721 N 6th Street in Fort Smith, Arkansas, an employee sustained an injury resulting in inpatient hospitalization. The employer failed to report the incident to OSHA within the twenty-four hour time period as required.
