If such hazards are present, or likely to be present, the employer shall:    a) On or about August 17, 2015, at the facility employees were assigned to clean-up a chemical spill of Power Bloc 15PT suffered nose, throat, and burning eye irritation during the clean-up period. The vapors and spray mist of Power Bloc 15PT were harmful if inhaled and could be absorbed through the skin. Vapor condensing on eyes could cause serious disturbances in vision with all layers of the cornea involved and may cause permanent eye injury. The employer's workplace hazard assessment did not include an assessment of chemical spills for chemicals such as, but not limited to, Power Bloc 15PT to determine the hazards of the clean-up activity and the personal protective equipment necessary to protect employees exposing them to inhalation and contact hazards.

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer's written respiratory protection program did not include procedures for the proper use of respirators during emergency situations such as, but not limited to chemical spills exposing employees to inhalation hazards.
