On or about October 1, 2015, during the installation of roofing materials on a new storage facility located at 48 Medina Line Rd, Sharon Center, Ohio 44274, the employer did not ensure the sub-contractors employees were protected from potential eye injuries while using an air powered nail gun.

On or about October 1, 2015, during the installation of roofing material at the job site located at 48 Medina Line Rd, Sharon Center, Ohio 44274, the employer did not ensure the sub-contractors employees were protected from a fall hazard of approximately 16 feet by utilizing a fall protection system/device.
