29 CFR 1910.146(c)(4): If the employer decides that its employees will enter permit spaces, the employer shall develop and implement a written permit space program that complies with this section. The written program shall be available for inspection by employees and their authorized representatives: (a) Employees are required to enter permit spaces when they do the task of "cleaning out behind the blade" on the garbage trucks they operate and the employer has not developed nor implemented a written permit space program thereby exposing workers entering them to caught between hazards.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a garbage truck such as "cleaning behind the blade" would not be exposed to caught between hazards.
