On or about October 01, 2015, at west side of the residence, employees were exposed to struck-by hazard because the forklift operator was not trained, evaluated and certified for the operation of the equipment.

On or about October 01, 2015, at west side of the residence, an employee was exposed to a fall hazard when ascending and descending from a rooftop on a ladder which was not extended at least 3 feet above the upper landing surface. The ladder was not secure at its top and a grasping device was not provided.
