29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 30, 2015, at Lot 49A, 1650 Azalea Gate Drive, Lawrenceville, Georgia:      a) An employee performing framing work was exposed to a fall hazard of approximately 15-feet while installing hurricane         clips from an approximately 6-foot A-frame ladder that was set at an unprotected window opening.       b) Five employees performing framing work were exposed to a fall hazard of approximately 11-feet when they worked           within 6-feet of an unprotected floor opening.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 30, 2015, at Lot 49A, 1650 Azalea Gate Drive, Lawrenceville, Georgia:      Five employees performing framing work were exposed to a fall hazard of approximately 11-feet when they used an      approximately 28-foot extension ladder that was not set three feet above the second floor landing.
