29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section. At the jobsite located at 8310 Outlook, Overland Park, KS 66207; employees engaged in residential roofing activities on a 18/12 pitch roof were exposed to fall hazards of approximately 9 feet 6 inches from the eave of the roof to the ground in that fall protection was not used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 8310 Outlook, Overland Park KS, employees using the metal extension ladder to access the roof to perform roofing activities were exposed to fall hazards of up to approximately 8 feet 5 inches from the ladder and roof to the ground below in that the side rails of the ladder did not extend at least three feet above the landing surface of the roof and was not secured to prevent displacement. The side rails extended 2 feet, 10 inches.
