29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Relocatable Power Taps (RPTs) were not used in accordance with listing and labeling in that: (a) At workstation 252 and to include workstations 253, two Multiple Receptacle RPT with Surge protection were connected in series to supply power to computer equipment, scanner, phone data, and printers. (b) At workstations 269, two Multiple Receptacle RPT with surge protection were connected in series to supply power to computer equipment, phone data, printers. (c) At workstation 275, three Multiple Receptacle RPT with surge protection were connected in series supplying power to desk lights computer equipment, phone data, printers and Lasko fan. (d) At workstation 276, two Multiple Receptacle RPTs with surge protection were connected in series and a third multiple receptacle RPT was connected in series to the RPT connected to the workstations outlet to supply power to desk lights, computer equipment, phone data, printers and Lasko fan. (e) In Netten office, a Multiple Receptacle RPT data port clamped to the desk was connected in series to another Multiple Receptacle RPT connected to the workstation's outlet to supply power to computer equipment, lamp, accessory equipment, and phone.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4: (a) At workstation 254, a Holmes air heater operated at 12.5 amps and was powered from a duplex receptacle through a 12 amp Relocatable Power Tap (RPT) thereby exceeding the rated capacity of the RPT which overloaded the relocatable power tap and the receptacle.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located had live parts normally exposed to employee contact: (a) In office 274, a modular workstation had an outlet receptacle that was missing the cover on the electrical tray where employees were exposed to live electrical parts. (b) In office 275, a modular workstation had two electrical outlet receptacles that were missing electrical covers on the electrical tray exposing employees to live electrical parts.
