29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment:     (a) Job Site - On or about October 1, 2015 the employer did not provide low voltage gloves rated for 1000 volts or less to employees doing electrical work on energized wiring rated at 277 volts.

29 CFR 1910.137(c)(2)(xii): The employer did not certify that electrical protective equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request to the Assistant Secretary for Occupational Safety and Health and to employees or their authorized representatives.       (a) Job Site - On or about October 2, 2015 the employer did not certify that the low voltage gloves given to employees to protect themselves from the hazards of electrical shock were periodically tested.

29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     (a) Job Site - On or about October 1, 2015 employees were allowed to break joints, dismantling electric parts, lights, and receptacles without de-energizing the electrical circuit or utilizing personal protective equipment, such as but not limited to, Class 0 rubber gloves rated for maximum use of 1000 volts.

29 CFR 1926.416(a)(3): Before work is begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken.  (a) Job Site - On or about October 1, 2015 the employer did not locate or identified electrical hazards to employees that were breaking joints and involved in demolition activities while working around energized electrical power circuits.
