29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: (a) Tait Towers Manufacturing, LLC, 401 West Lincoln Avenue, Lititz, Pennsylvania - Employees moved employer-made manual (human propelled) wheeled carts, loaded with various components of live event equipment, throughout aisles and doorways in the establishment. At times, the components of the live event equipment on the carts exceeded the safe clearance of doorways, exposing employees to hazards, including stuck-by and pinch points. On or about September 29, 2015, an employee sustained amputation injuries while moving a cart, whose load exceeded the clearance limit of the doorway opening between Rigging and Integration, causing the cart to strike the doorway opening and the load on the cart to shift and pinch fingers on the employee's left hand between the load and a support member of the cart. Corrected during the inspection.

29 CFR 1910.176(e): Clearance signs to warn of clearance limits were not provided where mechanical handling equipment was used: (a) Tait Towers Manufacturing, LLC, 401 West Lincoln Avenue, Lititz, Pennsylvania - Employees moved employer-made manual (human propelled) wheeled carts, loaded with various components of live event equipment, throughout aisles and doorways in the establishment. The doorways, including the doorway opening between Rigging and Integration, did not have clearance signs posted to warn of clearance limits, exposing employees to hazards, including stuck-by and pinch points. On or about September 29, 2015, an employee sustained amputation injuries while moving a cart whose load exceeded the clearance limit of the doorway opening between Rigging and Integration causing the cart to strike the doorway opening and the load on the cart to shift and pinch fingers on the employee's left hand between the load and a support member of the cart. Corrected during the inspection.

29 CFR 1910.242(a): The employer did not ensure that employees were furnished with tools and equipment that were in a safe condition: (a) Tait Towers Manufacturing, LLC, 401 West Lincoln Avenue, Lititz, Pennsylvania - The employer provided its own manual (human-propelled) wheeled carts for transporting live event equipment. Not all of these carts had handles or other safe means for employees to use to grasp the carts, so for carts that had no handles, employees grasped the metal support frames of the carts when moving the carts, exposing employees to the hazards of getting struck-by and pinched between a shifting load and a support member of the cart, on or about September 29, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.
