29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems. On October 01, 2015, at least five workers working on the roof were not protected at all times from falling by use of guardrail systems, safety net systems, or personal fall arrest systems. Paramount Builders Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA Inspection 908814, Citation Number 2, Item 1 issued on November 19, 2013, and final order on December 13, 2013 with respect to a workplace located at the STP Building, Atu'u, American Samoa.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On October 1, 2015, employees accessed the roof by using a job made wooden ladder that did not extend at least 3 feet above the upper landing surface being accessed, nor was it secured and a grasping device provided. Paramount Builders Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection 908814, Citation Number 1, Item 5 issued on November 19, 2013, and final order on December 13, 2013 with respect to a workplace located at the STP Building, Atu'u, American Samoa.
