29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 10/02/15 in the Die Cutting area; employer failed to protect employees from caught between hazards when they did not utilize a procedure for the control of the energy source(s) ( air and electrical/mechanical ) during service and maintenance activities, such as but not limited to, the removal and installation of the dies on the clam shell die cutter presses, greasing and lubrication of the presses. b) On or about 10/02/15 in the Die Cutting area; employer failed to protect employees from caught between hazards when they did not utilize a procedure for the control of the energy source ( air ) during the removal and installation of dies on the D'slugger machine c) On or about 10/02/15 in the Die Cutting area; employer failed to protect employees from caught between hazards when they did not document and utilize a procedure for the control of the energy sources ( air and electrical ) involved in, service and maintenance activities such as but not limited to, the removal and installation of dies, greasing and lubrication of the Preco die cutter press. d) On or about 10/02/15 in the Printing area; employer failed to protect employees from caught between hazards when they did not use a procedure for the control of the energy source ( electrical ) involved in removal and installation of the screens and set up on the M&R Saturn presses. ABATEMENT CERTIFICATION REQUIRED
