29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about September 28, 2015, the employer failed to report to OSHA an in-patient hospitalization of on an employee. The employer has failed to report an in-patient hospitalization to OSHA within 24 hours. This most recent occurrence of this took place at the facility located at 3200 Military Ave where on September 28, 2015 where the employer had an iron worker connecting steel beams and trim while working 12 feet above the ground below, the employee fell of the beam to the ground suffering fractures and internal injuries; the employee was taken by ambulance to local hospital where they received in-patient treatment as well as kept overnight for observation and continuous treatment. The employer did not report the incident to OSHA until October 2nd.
