29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) On or about 9/18/2015, the guard for the POWERMATIC chop saw was taped out of the way when an employees right hand came in contact with the saw blade. Three fingers (pinky, ring, middle) were taken off to the first knuckle by the saw blade. b) On or about 10/5/2015, The guard fingers were out of position for the Ironwood CUT 14 chop saw while it was being operated which would have permitted employee contact with the saw blade.
